Hydrocephalus: Understanding the Condition and Treatment Options
Hydrocephalus: Hydrocephalus is a neurological condition where an abnormal accumulation of cerebrospinal fluid (CSF) occurs in the brain. This
Dr. Dilip Kiyawat, after his MBBS in 1973 from MGM Medical College Indore (MP) achieved his postgraduate M. Ch. degree in Neurosurgery from KEM Hospital Mumbai in 1980. During this period he was trained under the able guidance of Prof. Dr Sunil Pandya and Prof. Dr. R D Nagpal. He submitted his thesis entitled “A Retrospective Study of 76 Cases of Brain Stem Tumours from 1950 to 1975” to Bombay University. He has wide experience operating and undergoing advanced training in various countries such as the United Kingdom, USA, Korea, Singapore, and Germany where he learned Micro Neurosurgery, Epilepsy Surgery, Stereotactic Surgery, Endoscopic Surgery and Minimally Invasive Spine Surgery. After 4 years of training at North Manchester General Hospital, Manchester UK, he returned to India in 1985 to join Ruby Hall Clinic Pune, Maharashtra. Simultaneously he worked as an Honorary Assistant Professor, and later as Honorary Professor of Neurosurgery for a period of 15 years at the B. J. Medical College and Sassoon General Hospital Pune. At both these hospitals he established Micro Neurosurgery Practice for brain and spinal disorders. Since 1998 he has been working as Consultant Neurosurgeon at the Jehangir Hospital Pune, and also at the Sainath Hospital Moshi since 2004. Along with other complex Neurosurgical operations, he embarked on surgical treatment of Epilepsy due to its high demand. For this he has visited centres in Liverpool UK, Cleveland Clinic USA, and Sree Chitra Tirunal Institute for Medical Sciences & Technology Thiruvananthapuram India. In association with his Neurologist colleagues Dr Nasli R Ichaporia and Dr Nandan Yardi, he was the first to start Epilepsy surgery in Maharashtra. With the experience of more than 6000 Neurosurgical operations during his long career of 45 years, Dr. Kiyawat has gained expertise in surgeries of Brain Tumours, Spinal Tumours, Surgery on Slip Disc (Sciatica), Degenerative Disorders of the Spine, Lumbar Canal Stenosis, Spondylolisthesis, Brain Injury, Spinal Injury, Surgeries on Craniovertebral Region, Epilepsy Surgery, Microvascular Decompression of Trigeminal Nerve for Trigeminal Neuralgia, Hydrocephalus, Endoscopic Brain and Spine Surgeries, and surgeries on Paediatric patients.
A young lady of 30 was brought unconscious following a two-wheeler accident. She was put on a ventilator to support her failing respiratory efforts. She lost all her brain responses and her pupils were not reacting (an ominous sign in brain injury cases). Her CT scan of the brain showed brain hemorrhages and brain contusions leading to an increase in intracranial pressure. Soon she would lose her life if left alone. Such patients can be salvaged by neurosurgery procedures to remove the blood clots and release the pressure from the brain. But here the signs of brain damage were far advanced to hope for any fruitful recovery. She had a high possibility of losing her life or remaining in a vegetative state for life which was still worse. Listening to all this her husband was in mental shock thinking about their son’s future who was only 3 years old. Her husband gave consent for surgery leaving all the consequences to God. The surgery was performed at night and lasted till morning. She showed favorable signs in her pupils and breathing within hours of surgery. She recovered rapidly and surprised all of us when went home 10 days later walking and in her all-normal faculties. In their own words, they were witnessing a God-sent personality to save her from the jaws of death. She became a normal household lady and also gave birth to a healthy baby girl a couple of years later.
23-year-old lady complained of severe headaches, vomiting, and blurred vision soon after giving birth to a healthy child. She was rapidly losing consciousness due to raised pressure inside her head due to a fast-growing tumor in her brain. MRI of the brain confirmed a large brain tumor in the occipital region on the right side of her brain. An emergency craniotomy was performed and the brain tumor was removed completely. She became fully conscious had no neurological deficiency and was discharged within a week. At three years follow up she is doing well and looking after her growing son efficiently.
22-year-old lady started getting fits (convulsions) at the age of 14. She left school due to fits and poor performance. Her frequency of fits was 2-3 / day. She took antiepileptic medicines for years in various combinations from various neurophysicians. None could control her fits. With the help of our Neuro physician Dr Nasli Ichaporia and Epileptologist Dr Nandan Yardi, she was investigated with EEG and An abnormal epileptic focus was identified in her brain. The surgery was performed by Dilip Kiyawat where he explored the abnormal epileptic area and excised it. Postoperatively she was completely free from fits and was on small doses of a single anti epileptic medicine.
Here again, many elderly patients come with agonizing pain due to trivial falls at home causing osteoporotic vertebral collapse. They get a “new life” after vertebroplasty.
76-year-old lady was in severe agony due to a backache that had started after a fall at home. This worsening backache compelled her to remain at home and for the last couple of weeks she was unable to sit or even turn in bed. She reluctantly came to the hospital and did not allow the doctors to examine her for fear of pain. Her X-ray and MRI investigations showed a compression fracture of one of the lumbar vertebrae. Her bones had become brittle due to age, age-related hormonal changes, and calcium depletion leading to osteoporosis and rendering her bones vulnerable to easy fractures even after a trivial fall. The aim of treatment here is to make her osteoporotic vertebra stronger by injecting bone cement into it. Dr Kiyawat performed this procedure called vertebroplasty which is performed under local anesthetic and X-ray control. A needle is inserted into the affected vertebral body and bone cement made of Polymethyl Methacrylate (PMML) is injected into the affected vertebral body under X-ray control. In the immediate postoperative period, she was relieved of her excruciating pain and started walking so quickly that she couldn’t believe such a miracle could occur. She was seeing a (in her own words) a “reincarnation of God in the form of a doctor.”

One of the best doctors in Pune. Very polite and friendly, down to the earth. Listens every single thing from patient as well as relatives queries. My best experience was for disk bulging of my relative for whom sir suggested traction from front and back which avoided spine surgery. Hats off. May god give you long, happy and healthy life.
